Training & Office Hours
We offer training every 2nd and 4th Tuesday of the month from 12-1:30PM (Zoom link). Register for trainings.
REDCap questions can be answered during office hours (every Wed 3:00-4:00pm and Fri 9:00-10:00am [Zoom]).
How to complete the service intake form

- Fill NextGen Intake Form: This is common form to request multiple services at once. This form collects generalized information.
- Service Specific Form/ Forms: Based on what services you select under ‘Requesting for:’ service specific form will be displayed. Fill out all the displayed forms to complete the request.
- Faculty endorsement required for staff/residents/students to fulfill requests.
